Please download, complete and return the Financial Report and Final Claim Form (as a Word document) by Friday September 16 to claim your final HAF payment in respect of a Summer 22 Fit for All project.
The form consists of six parts:
- a table showing how HAF funding was spent – against how it was projected to be spent in the project proposal form
- a space to explain any variation in the level of HAF funding
- a space to explain any variation in the way HAF funding was spent across different types of cost, eg staff costs, venue costs etc
- a table showing the total funding including from non-HAF sources of the project
- a space to add notes on value for money
- statement, personal details and signature.
